It is fair to say that the growth in cosmetic dentistry in this country over the last 10 years has been astronomic. Dentistry is a multi-billion pound industry and cosmetic dentistry is the fastest growing part of it. Helping someone enhance their smile is a wonderful part of our job and their is no doubt that it can transform peoples lives. Sounds live there’s a “but” coming, and there is. It can be great to do “but” we need to consider every aspect of the patients dental health before we embark on any changes.
This case highlights a problem, not with the veneers per se, but with the fact that they were put in a mouth which suffers great damage from a clenching and grinding habit and in a bite that had no room for them. The result was multiple de-bonds with the veneers re-cemented so many times that they became chipped, stained and poor fitting. At StoneRock we were able to correct the problem by adjusting the patients bite (with direct white fillings onto the backs of the front teeth) so they were protected from further damage from the grinding and their was sufficient room created to be able to fit well retained crowns. Fortunately we work with a wonderful technician, Ross Liversidge at Precedental Laboratory in the Midlands who was able to match the porcelain beautifully.

The previous case showed just how effective non-invasive techniques can be. I never get tired of doing this type of dentistry and I hope I never will. The world of bio-aesthetics has shown dentist how we can make dramatic changes to a patients teeth with little or no alteration to the underlying sound tissue. The fact that we can do this at the same time as protecting the teeth from further damage makes this a genuine “win-win” technique. The whitening takes 2 weeks to complete and the builds up are done in one visit, making this not only the cheapest and least invasive technique to change your smile but also the quickest.
This case was one that we monitored for three years until we were able to see that the damage to the teeth was on-going at which point we decided to build up the teeth, having whitened them first. As a matter of course in all such cases we discussed alternative techniques, which for this case would have been multiple veneers and crowns which the patient preferred not to have.

This case highlights the point made in the last post about cost effective alternatives. The tooth in question had be scheduled for extraction and could only be replaced by an implant retained crown (approximately £2000) or a denture. A denture would be a problem in this case as there was active gum disease which is always harder to control in patients who where dentures. The tooth was too broken down for a routine crown.
At StoneRock we were able to offer an alternative option of a fibre reinforced composite to rebuild the tooth. This option is not only the cheapest and quickest option but it is the least invasive for the patient. (The cost of £275 is less than half the cost of it’s nearest rival). Clearly this is not a tooth for life and we will be faced with the loss of this tooth at some time, a fact the patient is fully aware of. If we can delay the loss of the tooth for 5-8 years then I believe we have done a great service to the patient and have allowed them time to plan their finances and, of course, allowed technology some time to improve and offer perhaps yet more alternatives.
